During the National Day last year, laboratory technicians were on a shift, but all the physical examination items on the report had results."
So the inspection team immediately convened a meeting and arranged for the inspection team's cadres to review the national medical examination files of the health center one by one. After two days of comparison and analysis, it was found that there was indeed a problem of fraud from the files.
To further find out the truth, the inspection team verified the staffing situation of the laboratory of the health center and determined that the laboratory of the health center had only one staff member who was on a shift during the National Day last year and was not on duty.

Subsequently, the inspection team retrieved video surveillance images from the laboratory and found that the laboratory technician was not on duty during National Day. The urine collected by the health center was dumped directly into the trash can without sampling for testing.

However, the staff of the health center also need to take vacations. In the case of insufficient professional talents and no one to shift work, they must be allowed to work. Personnel working overtime may suspend the work of public health and family doctors, but this affects the progress of the completion of the indicators, so health centers are often unable to achieve both.
Especially at the time of the inspection, the indicators must be completed and the doctors and nurses are too busy, so the fake phenomenon of "pouring the urine into the trash can" will naturally occur.
However, strengthening the allocation of professional talents cannot be achieved overnight. It requires many changes at the grassroots level. Remuneration and the future are all immediate issues. Especially in poor and remote areas, it is even more difficult to retain talents.

As early as July 2018, the Commission for Discipline Inspection of Yongding District, Zhangjiajie City, Hunan Province notified a health center of medical examination fraud.
The investigation team obtained the health records of the elderly in the health center and found that the test results of alanine aminotransferase, aspartate aminotransferase, triglyceride, and other tests items in the blood test reports of many people were the same. After checking, it turned out that the same blood test report was registered under multiple names.
After the facts were ascertained, the Yongding District Health and Family Planning Bureau gave Wang Jun a serious warning, Li Yuquan, a senior file management specialist, and Fan Hongxiang, the head of the public health team, dismissed Huang Tao, the director of the functional department. Four related personnel were dealt with by interviews and exhortations.
After understanding, this hospital needs to collect more than 1,000 blood samples in just a few days because the inspection time is approaching, which is almost impossible to complete. Because there is only one biochemical analyzer in the hospital that can test the collected blood samples, only about fifty or sixty blood samples can be tested every day.
As a result, they collected a total of 1,004 blood samples from the elderly in just 8 days. Only 292 blood samples were tested. The remaining 712 blood samples were directly treated as medical waste without testing. Falsified treatment on the medical examination list.
With insufficient staff, insufficient equipment, tight time, and heavy tasks, the health center eventually "committed crimes." The behavior is wrong, but the reason behind the behavior is worth thinking about.
Some indicators are too high:
Divorced from reality and spawned fraud
As we all know, public health and family doctors have index requirements for each contract. Through continuous improvement of policies in recent years, the index setting has been relatively reasonable, and the phenomenon of data fraud has been further reduced. However, in a few areas, there are still phenomena that index setting is out of local reality.
Some jurisdictions have a large population and it is relatively easy to complete the indicators, but if they are concentrated for some time, they will face the pressure of time-critical tasks; some jurisdictions have a small population, although the tasks are not heavy, the indicators are difficult to complete.
